NIH continues to transition grant mechanisms from PHS 398 to the SF424 forms and electronic submission through Grants.gov. For grant mechanisms that are still submitted on paper, the PHS 398 forms is used. Complete instructions and NIH forms may be found at the NIH Forms & Applications website.
