Institutional Routing Form


The Institutional Routing Form facilitates compliance with federal, state, sponsor, and university policies. Each RU Principal Investigator (PI) is required to submit a Routing Form before their applications/reports may be submitted to the sponsor by OSPA on behalf of the institution. Projects with multiple RU PIs and/or with work to be performed at multiple RU laboratories require a Routing Form from the project PI (and their HOL if not the same person), as well as from each of the other RU HOLs whose laboratories are on the project. The Routing Form is often the only document signed by the PI. OSPA cannot process an application/report/other submissions for the PI without this authorized sign-off.   

The institution, via OSPA, may approve and submit a sponsored funding application/technical report only after the required Institutional Routing Forms are completed and signed by the PI, by his/her HOL (in cases where PI is not the HOL) and, when needed, by University senior officials.

The form assists and protects investigators and the institution by enabling the systematic, streamlined and cross-departmental oversight of compliance areas such as human and animal subjects work, laboratory safety, and others as needed. Used to document mandatory assurances for all proposals and other activities involving sponsored support, the form can also serve as a checklist of regulatory requirements for investigators and for institutional reviewers.

OSPA periodically reviews and updates the Institutional Routing Form to capture any new regulatory and compliance requirements and/or modifications of existing requirements. This is done in cooperation with all the institutional compliance areas. 

Be sure to save the form on your computer PRIOR to entering any data and note that copying and pasting into the form may corrupt it!  Always use the latest version of the Routing Form.
