How do I complete the “S2S Forms” tab in InfoEd?

Click edit on the “PHS Fellowship Supplemental Form”.  Once opened, complete the form per the SF424 instructions.  This includes uploading attachments such as the Introduction (if applicable), Specific Aims, Research Strategy, etc.   When finished, click “Completed” in the upper right corner.

The PHS Fellowship Supplemental Form asks for a list of "Applications for Concurrent Support". Is this where I need to list all the grants that I applied for during this time period, or is this only asking about concurrent NIH support?
All applications that have been submitted (or are planned for submission) that have an overlap of project dates with the NIH fellowship must be identified.

What do I enter in “Section E. Budget” of the PHS Fellowship Supplemental Form?
In the budget section of the PHS Fellowship Supplemental Form asks if you are requesting funds for Tuition and Fees.  You should either indicate “None Requested”, or check off “Funds Requested” and fill in the appropriate amounts for each year.

How do I enter the budget?
The NIH Individual Fellowship applications do not contain a standard budget.  In the “Summary Budget” tab in InfoEd, simply enter the total amount requested for each year in the “Other Costs” tab.  This amount is a sum of figures (stipend level, institutional allowance, and any tuition and fees) as set by the NIH (per NOT-OD-14-046).

What should I request for tuition and fees?
Predoctoral applicants should contact his/her SPO to obtain the annual tuition amount as set by the Dean's Office and Finance.  Postdocotoral applicants should only list tuition and fees for any courses taken outside of Rockefeller University. 

Do I need to include a Budget Justification?
No, a budget justification is not required.
